I agree to have Lash Lift solutions applied to my natural eyelashes and/or removed and retouched.

By electronically signing this agreement, I consent to the Lash Lift by the certified Lash Artist.

I understand there are risks associated with Lash Lifts/coatings applied to or removed from my natural eyelashes, including the eye itself, and could result in stinging, burning, blurry vision should the adhesive enter the eye or should an allergic reaction occur.

I understand that some irritation, itching or burning may occur on the skin if the bonding agent comes into contact with it.

I understand that if the solutions come into contact with my eye, my eye will be flushed with water and I will be assisted in seeking medical attention immediately.

I understand that even though the certified Lash Artist applies or removes the Lash Lift solutions using the proper technique, the instruments/tools, tapes, cleaners, eyelash gel pads, adhesives and removers used may irritate my eyes or require a medical professional treatment/care and subsequent removal of the eyelash extensions.

I understand Lash Lifts may not be suitable for everybody and that it is not uncommon to have allergic irritation/reactions to the adhesive, tools and products used.

I agree if I experience any of these medical conditions with my lashes I will contact the certified Lash Artist and consult a medical professional at my own expense.

While there are no refunds for this service once applied, reversal notified within 7 days and completed within 14 days is complimentary. Notification after 7 days, or completed after 14 days a $50 fee will apply.

I understand that this is a semi-permanent procedure, and my natural lashes will continue to grow and fall out normally, making re-lift appointments necessary to maintain the original look achieved. Most clients require a re-lift appointment every 3-4 months. To secure an appointment time that suits you it is necessary to book your infill appointments well in advance.

I understand that the Lash Artist has a 48 hour cancellation policy and all NO SHOW appointments will be charged 50% of their appointment.

I understand that a 50% deposit is required to secure my Lash Lift this is a non-refundable deposit but your appointment can be transferred to another day with at least 48 hours notice.

I release my Lash Artist from all liability associated with this procedure, which is performed with the utmost attention to safety and proper application using tools and products the Lash Artist has been professionally trained to use. There are no guarantees for the lift time length of the Lash Lift.

I do not hold Lash Artist responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed today.

I also agree that I have removed and indemnify Lash Artist from any loss of the said contact lenses and any such cost associated with their removal by me.

Permission is granted to take before and after photos of my eyes/face which may be used for marketing purposes on social media, website and within the salon.

I understand that it is imperative that I disclose all of the information requested in the Client Waiver and should any information provided change, Lash Artist is informed immediately.

I have cited all conditions and circumstances regarding my health history, medications being taken, and any past reactions to products or medications.

I understand that additional conditions could occur or be discovered during the procedure which could affect my ability to tolerate the procedure.

I understand that if I have any concerns, I will address these with directly and privately with Lash Artist .

If I am unhappy with the service provided by Lash Artist I will provide feedback promptly and in a kind and private fashion. Lash Artist will attempt to rectify any negative feedback.

In the event I may have additional questions or concerns regarding my treatment, I will consult Lash Artist immediately.

I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures.

I understand this agreement will remain in effect for this procedure and all future procedures conducted by the certified Lash Artist.

I read English and understand that this consent agreement is legal and binding.I certify that I have read, and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered.

For new clients, please fill out the client form.

UNDER 18 - Please provide written confirmation from your parent or legal guardian and a telephone number to contact on the day.

A non-refundable 50% fee is due upon booking (Booking Fee) for all new clients.
